If you are a family living in Lyon County in need this holiday season, Heart to Heart would like to offer you a gift of food for the Christmas holiday. In addition, if you have children age 18 and under living in your household, we can also provide gifts to make their Christmas holiday happier!

Heart to Heart distribution will be at the Marshall National Guard Armory on SATURDAY, DECEMBER 15, 2012 from 10 AM to 1 PM. We need to know how much food and how many gifts we will need, so you must return this completed White form in the enclosed envelope NO LATER THAN DECEMBER 1, 2011.

You will receive a postcard notifying you that Heart to Heart has received this registration form. You MUST bring this postcard verification with you to the Armory on DECEMBER 17, 2011. Delivery will be available only to disabled and elderly when requested prior to DECEMBER 1, 2011. You must call 507-532-4126 and leave your name, address, and phone number to request delivery.
